I understand Cater Wellness does not treat medical conditions nor does it claim or guarantee to treat or relieve any medical condition.

-Cater Wellness should not be used on those with cardiac issues.
-Cater Wellness should not be applied across or on the thoracic cavity.
-Cater Wellness should not be applied over carotid sinus nerves.
-Cater Wellness should not be applied over inflamed, infected, or swollen areas of the skin.
-Cater Wellness should not be applied over or near cancerous areas.

I understand that any procedure involves risk. On a rare occasion risks may include redness, swelling, burns, bruising, irritation, skin reaction, or increased heart rate.

I have been honest and forthright about my medical history, and am healthy to use the equipment required for Laser Lipo, emsulpt, Lipo Cavitation, Cryolipolysis machine, lip fillers, radio frequency for skin tightening and/or vacuum therapy. I am not pregnant, have any metallic implants (including a pacemaker), nor any other disease or condition that may be negatively impacted by the body contouring devices.

Acknowledgement: I understand each person has a different response to the body sculpting & body therapy treatment. The risks, benefits, and possible results have been explained to me. I have been provided the opportunity to ask questions and received satisfactory responses.

I voluntarily provide my consent to partake in the body sculpting & body therapy treatment. Should any pain or discomfort occur I will immediately notify the specialist who performed the procedure. I will not hold the specialist or Cater Wellness liable for any irritation or-desired effects.

It is critical that the dietary and lifestyle changes are made to help support the goals of treatment. To ensure optimal results, we highly suggest drinking lots of water (before and after treatment), brisk exercises (walking, swimming, jogging, or aerobics) for a minimum of 30 minutes daily. Eat healthy.

I consent to the taking of photographs and authorize the use of them for results, education and promotion.

This agreement will remain in effect for this procedure and all future follow-ups conducted by the specialist. I read English and understand that this consent agreement is legal and binding. I have read and fully understand all information in this agreement. I am over 18 years of age and consent to the agreement and to the Cater Wellness treatment procedure.
